If you wish to apply for this card you will have to provide 2 forms of identification, one for name and address (utility bill or credit card statement (no more than 6 weeks old) or drivers licence) and one for signature and name (credit card, cashline card, cheque guarantee card).  You must also provide us with  your bank details including the sort-code and account number.
